Shift: 1  

The role of this pharmacist is to provide medication management in a series of potential disease states including (but not limited to) cardiology, endocrinology, fertility, gastroenterology, hepatology, infectious disease, internal medicine, neurology, oncology, pulmonary, rheumatology and/or solid organ transplant.

Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department’s quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services. Other duties as assigned.

A minimum of one year hospital or three years of other pharmacy experience is preferred.

Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred.

Active Wisconsin Pharmacist License is required or eligible for and must obtain within 3 months.

Other information:

The specialty pharmacist provides medication management which helps in the coordination of all patient medications in an office setting by:

  • Reviewing patients’ prescription(s) and over-the-counter products

  • Answering all of patient medication- related questions

  • Ensuring clinical appropriateness and effectiveness of each medication through review of our electronic medical record, clinical resources and pharmacy dispensing platform

  • Providing intensive adherence management and demonstrating improved patient outcomes

  • Assessing for and providing strategies to minimize side effects.

  • Ensuring patients understand the purpose, administration, and storage of each medication

  • Communicating with internal and external healthcare providers about patient side effects, new prescriptions, and/ or refill requests

  • Reviewing and assisting with prescription insurance and financial concerns

  • Identifying what needs to be refilled, synchronizing patient medication fills and establishing shipment and delivery dates to prevent missed doses 
    Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred.
