POSITION SUMMARY:

 

The
Pediatric  RN/LVN night nurse , provides
coordination of service for patients with necessary medical nursing and ancillary
staff.  The Pediatric Case Manager
coordinates special health care services and programs such as the Medically Dependent
Children Program (MDCP) and Comprehensive Care Program (CCP) within an assigned
territory.  The Pediatric Case Manager implements
policies, promotes JHS Team initiative, oversees and coordinates with scheduling,
and coordinates program activities with Texas Department of Human Services’
(TDHS) staff.  Other duties include
promoting and maintaining favorable relationships with staff, doctors,
discharge planners, social workers, patients/clients and their families and the
public.  The Pediatric Case Manager
continually analyzes and evaluates all provided services and programs, quality
and continuity of patient/client care, and staff utilization seeking methods
and ways to improve the delivery of care and services to patients/clients.  The Pediatric Case Manager may occasionally
work individual case assignments but primarily functions as intermediary
between Pediatric Director of Nurses and field staff.

 

Key Accountabilities:

 

Client Care: (component proportion 30%)

  • Supervises
    and coordinates nursing and therapeutic services to assigned clients.

  • Manages
    JHS approved and established directives for promoting continuity of care from
    pre-admission to post-                                             discharge activities.  

  • Reviews
    POC, discharge summary, and verbal orders for appropriateness and application
    of physician’s plan of care.

  • Conducts
    and/or approves Initial Assessments for clients.

  • Initiates
    hold orders and performs post-hospital assessments as required by regulation.

  • Takes
    referrals, performs admission assessment visits and evaluates for Agency
    eligibility.

  • Implements
    and manages JHS administrative and clinical procedures and policies and TDHS
    programs directives.

  • Reviews
    patient/client records, as necessary, for accuracy and quality.

  • Monitors
    charting of client records for Texas Department of Human Services’ regulations
    and program compliance

issues.

  • Takes
    on-call duty on nights, weekends and holidays as assigned.

  • Other
    duties, as assigned, including skilled visits.

 

Staffing:  (component proportion
50%)

  • Coordinates
    case load distribution of assignments.

  • Provides
    leadership in coordinating, promoting and maintaining high standards for the
    delivery of quality care to the patient/client by the assigned Care Team.

  • Assures
    compliance with attendant and LVN supervision.

  • Identifies
    and coordinates the need for involvement of other members of health care team.

 

Administration: (component proportion 15%)

  • Promotes
    a positive professional working relationship with physicians, pharmacists,
    social workers, other professionals in healthcare disciplines.

  • Maintains
    awareness of State and Federal guidelines and revisions for home health
    coverage and compliance.

  • Maintains
    client, employee and agency confidentiality.

  • Attends
    mandatory meetings and in-services.

  • Complies
    with agency Human Resources requirements.

  • Completes
    reports and provides other necessary evaluations as directed.

 

Marketing / Public Relations: (component proportion 05%)

  • Promotes
    company and its services through positive job performance, by building good
    relationships, and by actively participating in professional and local
    organizations and community projects.
